Aims of the Project

The aim of the paper is to help the society know how cardiac rehabilitation focuses on patient education, individually tailored exercise training, modification of the risk factors and the overall well-being of men of over 50-years.  Furthermore, to enumerate how utilization of better lifestyle management can better the life of cardiac men in a positive manner. All the proposed objectives are to be instigated through offered promotion programs as the primary strategies that can counter cardiac among men of over 50 years in Australia.

Specific: Plan to educate the community the relevance of cardiac rehabilitation and lifestyle management for men of over 50 years.

Measurable: Use of the monthly Physical Activity Tracker to keep track of the number time a cardio exercise time frame and the strength training attained on a weekly basis. 

Achievable: Exercise 30 minutes every day six times a week.

Realistic: Start running every day for a mile and eat healthy food and fruits respectively for better health results.

Timely: Possibly complete the exercise within a time frame of six months.

Unmet Community Health need for Men of over 50 years

O’Loughlin, Browning, and Kendig (2017) demonstrate that men who are over 50 years need to be taken care of, so as to ascertain that they live a better and healthier lifestyle. Just like any other individuals in the society, men of over 50 years need to be taught on the relevance of rehabilitation and lifestyle management especially following a myocardial. The community needs to be addressed on the need to relate to them the relevance of cardiac rehabilitation and lifestyle management following a myocardial. It will ascertain that better strategies are undertaken to improve their way of life.

A cardiac myocardial infarct is a very dangerous disease especially among men of over 50 years, thus the need to notify the community of the relevance of rehabilitation and better lifestyle management strategies. The problem is brought about by the negligence of taking part in physical activities meant to counter myocardial infarct disease. The inability to take part in physical activities is an entity strongly associated with increased risk of heart disease among men of 50 years and over in Australia (Cohen et al., 2013). According to the Heart Foundation (n.d.), heart disease is the single leading cause of death in Australia with 19,777 as of 2015. The Heart Foundation (n.d.) further indicates that as of 2014, awareness of the heart-related disease as the leading cause of death among men is higher when compared to their female counterparts. The above chart portrays that men are often affected with cardiac diseases from the ages of 35 years and above. At 35-44 years, the effects are not high in men. On the contrary, at the age of 45-54, the increase in the effects of the disease is noted on the chart. As one grows older, the chart indicates that men are often at risk. Thus, illustrating to the community on how the disease often affects them in terms of age will help them better manage their lifestyle especially when associated with cardiac disease (Antman, 2012).

The primary causes of cardiac myocardial are smoking among men as a way of relaxing or leisure related aspects. Lack of utilizing fruits in their daily meals further leads to an increase in the disease within the male individuals in Australia. Furthermore, consumption of alcohol is another contributor of cardiac myocardial among men aged 50 and above in Australia. It is apparent that men of 50 years and over often use alcohol both in their homes and the club as a source of quenching their thirst. Stanner and Frayn (2008), indicate that obesity is an aspect that contributes to cardiac myocardial among men in Australia due to the inability to live healthy lifestyles. Facebook and Twitter need to be implemented within a time frame one week.  Based on the fact the young people often use the internet, they will be able to share information with the older generation easily and appropriately (Anderson & Deravin-Malone, 2016). Community programs will entail organizing a discussion on living healthy and the relevance of rehabilitation programs for men aged 50 years and over ones a month. The move will help the community, and especially men aged 50 years and over with cardiac diseases. Fundamentally, the proposed programs are both associated with modernity and ease, thus, will generate positive results both within a short and long-term point of view.

As a result, Facebook and Twitter will share pictures, videos, and articles on what would lead to a 50-year-old man have myocardial infarct and what strategies can be incorporated to counter the situation. The online platform will encourage family members to work with victims of the disease as a way of enumerating to the patients that they are cared for and loved. The use of the internet will, therefore, ascertain that better results are attained because the approach will further encourage those reading, watching, and commenting on what they see online to share with other members.

The aim of the proposal is to come up with promotion programs that will help over 50 men following myocardial infarct to take the needed strategies like rehabilitation and better lifestyle management as the primary route to better healthy living. So as to fully instigate the proposed aim, the principle of respect for the values, traditions, and beliefs of patients will have to be incorporated. It is where affordability and availability of the services will be incorporated to the latter via the community programs and the online proposed strategies as an approach to better service. Furthermore, utilization of the teamwork relationship with co-employee and members of the health team is essential for accomplishing the objectives of the nursing sector (LeMone et al., 2014). The implication here is that community members, nurses, family members, friends and the myocardial infarct patients will need to work together to instigate the contents of the proposed strategies effectively.  Fundamentally, the contents of the proposed objective will be incorporated with nursing principles to fully attain the objectives of cardiac rehabilitation and lifestyle management of men of over 50 years following myocardial infarct.

Teamwork and communication are strategies I would recommend as platforms that will ascertain that men of over 50 years are enlightened on the importance of rehabilitation and life management following myocardial infarct. Thompson (2010) suggests that the use of teamwork will entail encouraging members, i.e. victims, friends and family members to assist where necessary. It is an appropriate strategy because it brings all members to with the objective of helping each other financially and emotionally. The outcome of the strategy is, therefore, better recovery and reduced cardiac diseases among men of over 50 years because they are made aware of what the disease entails thus better lifestyle management approaches.

Use of the communication approach is also essential because it consists of interaction among members both offline and online. Analyzing of the Aims

The evaluation of the project will be undertaken via surveys, Facebook and Twitter as well. Surveys will consist of a questionnaire where participants who will attend the community program will be tasked with the objective of answering the questions. Each form will be evaluated in a percentage so as to know the impact of the discussions and the benefits being attained through the proposed promotion strategies.  Kelly and Fuster (2010), show that Facebook, on the contrary, will entail the number of people who have liked the group on monthly basis as a communication approach of the effects of the discussion undertaken. Lastly, Twitter will be analyzed on the basis of the number of people following the twitter handle on a monthly basis as well.
